Mechanism of action

Under the influence of gastric juice, insoluble compounds are formed from bismuth tripotassium dicitrate, which are deposited mainly on the ulcerated area and inhibit the activity of pepsin. Bismuth tripotassium dicitrate also protects the mucosa by stimulating the synthesis and secretion of endogenous PGs, thereby increasing the production of bicarbonate and mucin. In addition, tripotassium bismuthate dicitrate has antibacterial activity against Helicobacter pylori. The eradication of these bacteria is accompanied by an improvement in the histological picture and symptoms of peptic ulcer disease.


Tripotassium bismuthate dicitrate promotes the healing of a significant proportion of gastric and duodenal ulcers. Its antibacterial effect is associated with a lower incidence of ulcer recurrence in the first year after cessation of treatment compared to some other drugs.


Bismuth tripotassium dicitrate has a local effect. However, a small amount of bismuth is absorbed (less than 0,2% of the dose) during therapy.


Bismuth is distributed predominantly in the kidneys. Only trace amounts may be found in other organs.


Bismuth tripotassium dicitrate locally precipitates in the stomach under the action of gastric juice, forming insoluble compounds, presumably bismuth oxychloride and citrate.

The bulk of ingested bismuth is excreted in feces. The renal clearance of small amounts of absorbed bismuth is approximately 50 ml/min. At least a three-component model is required to describe bismuth excretion over time. T1/2 is 5–11 days.

Preclinical safety data

Single oral administration of bismuth did not increase mortality in rats at doses up to 2000 mg/kg. There were no significant changes associated with bismuth administration in clinical signs, body weight, food intake, hematology, clinical chemistry, urinalysis, organ weights, necropsy, or histopathologic findings in a 28-day repeated oral dose toxicity study. The level of no observed harmful effects of bismuth was determined to be 1000 mg/kg for males and females. No signs of hepatotoxicity were observed. There were also no histopathological changes in the bone marrow or lymphoid organs (thymus, spleen, lymph nodes).

The mutagenicity of bismuth cannot be assessed due to insufficient studies.

Evidence-based studies of the effect of bismuth citrate on male or female fertility and early embryonic development have not been conducted. Maternal toxicity was detected in rabbits. However, no adverse effects were observed on pre- or post-implantation loss, number of viable fetuses, or fetal development. Bismuth tripotassium dicitrate has not been evaluated for phototoxicity.

Application in pregnancy and lactation

There is insufficient data to assess the potential adverse effects of tripotassium bismuth dicitrate during pregnancy in women. To date, animal testing has shown no evidence of such effects. Due to the lack of data, the use of bismuth tripotassium dicitrate during pregnancy is not recommended.

There are insufficient data on the use of bismuth tripotassium dicitrate during lactation in women to evaluate its potential harmful effects during breastfeeding.

Symptoms: An acute, severe overdose can lead to the development of renal failure with a latent period of up to 10 days.

Treatment: a single exposure to a very high dose should be treated with gastric lavage followed by repeated administration of activated charcoal and osmotic laxatives. Typically, this will prevent bismuth absorption to the point that additional treatment is not required.

In both cases of acute and potentially chronic intoxication, to confirm the effect of bismuth, it is necessary to determine its concentration in the blood and urine. If symptoms are caused by acute or chronic bismuth overdose, chelation therapy with dimercaptosuccinic acid or dimercaptopropanesulfonic acid should be considered. If there are also signs of severe renal dysfunction, hemodialysis should be performed after chelation.

Dosing and Administration

Long-term use of high doses of bismuth compounds is not recommended, because this may lead to the development of reversible encephalopathy. The likelihood of this event occurring is very low when bismuth tripotassium dicitrate is used as recommended.

Concomitant use of other bismuth-containing compounds is not recommended.

Impact on the ability to drive vehicles and operate machinery. There is no data on the effect of the use of bismuth tripotassium dicitrate on the ability to drive vehicles. In any case, the effect of bismuth tripotassium dicitrate on the ability to drive a car or operate machinery is unlikely.
